Volt50 Liability Waiver & Release

1. Assumption of Risk

I, the undersigned, understand that participating in physical activities (including but not limited to [Yoga/Pilates/Barre/HIIT/ShredFast/Personal Training] involves inherent risks of injury. These risks include, but are not limited to, muscle strains, falls, or cardiovascular stress. I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in the programs.

2. Medical Representation

I confirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in these classes. I acknowledge that it is my responsibility to consult with a physician prior to and regarding my participation in any fitness program.

3. Waiver of Liability

In consideration of being permitted to participate in the classes at Volt50, I hereby knowingly, voluntarily, and expressly waive any claim I may have against Volt50, its owners, instructors, and staff for any injury or damages that I may sustain as a result of participating in the program, except in cases of gross negligence or willful misconduct.

4. Studio Policies & Safety

● Arrival: I agree to arrive at least 5 minutes before class. Late entry may be denied for safety reasons (missing the warm-up).

● Equipment: I agree to use all studio equipment as instructed and will alert staff immediately if I notice any equipment malfunctions.

● Property: The studio is not responsible for the loss, theft, or damage of personal belongings brought onto the premises.

5. Consent to Emergency Treatment

I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during my time at the studio.

Specific acknowledgment

1. Access and Premises Disclaimer

I acknowledge that the entrance to Volt50 involves navigating steep external stairs. I represent that I am physically capable of using these stairs safely. I agree to exercise caution at all times when entering or exiting the building, particularly during inclement weather. Volt50 accepts no liability for slips, trips, or falls occurring on the access stairs due to improper footwear or failure to use the provided handrails.
